Terms and Conditions

In consideration of the services of Thrive Well, its agents, owners, volunteers, participants, employees,
and all other persons or entities acting in any capacity on its behalf (hereinafter collectively referred to
as “Thrive Well”) consumers hereby agree to release and discharge Thrive Well, on behalf of themselves,
their children, their parents, their heirs, assigns, personal representative and estate as follows:

1. Any purchase/use is made understanding and accepting these terms and conditions, consumer will
not hold Thrive Well responsible for any injury, illness, allergic reaction, or lack of results while engaging
in use of any products or at any time in the future. Consumer acknowledges that Thrive Well is NOT a
physician, licensed dietician, certified personal trainer, or any other licensed/certified professional.
Consumer completely acknowledges that they are simply receiving advice and that it their choice to
adhere to the provided advice. Consumers’ use of products sold by Thrive Well is voluntary, and by
making a purchase the consumer accepts responsibility for any harm, injury, illness or death that may
result from my participation.

2. Consumer understands that it is their responsibility to consult with a physician before using any
products offered by Thrive Well. Consumer hereby affirms that they do not suffer from any mental or
physical disability which would prevent them from making the conscious and informed decision to use
hemp derived products.

3. Consumer hereby voluntarily releases, forever discharges, and agrees to indemnify and hold harmless
Thrive Well from any and all claims, demands, or causes of action, which are in any way connected with
my participation in the use of products sold by Thrive Well, including such claims which they, their
children, parents, heirs, assigns, personal representative and estate have or may have that allege
ordinary negligent acts or omissions of Thrive Well.

4. Any advice regarding dietary supplements provided by Thrive Well is strictly done so by opinion only,
and these products may not have been approved by the FDA. Any companies or products mentioned by
Thrive Well are not affiliated with Thrive Well and Thrive Well is not liable for any negative
repercussions. By making a purchase or consuming products, consumer agrees to these terms and
conditions and will not hold Thrive Well accountable for any issues, health related or nonā€health related
that may result from consuming a product suggested or recommended by Thrive Well. Consumer
understands that they are responsible for understanding their own body and the health risks involved in
consuming a dietary supplement or any hemp derived products offered by Thrive Well.

5. Consumer agrees that the foregoing liability waiver and assumption of risk agreement and terms and
conditions is intended to be as broad and inclusive as is permitted by the law of the Commonwealth of
Kentucky and that if any portion thereof is held invalid, it is agreed that the remaining provisions shall
continue in full force and effect. Likewise, consumer agrees that if legal action is brought, it must be
brought in Christian County, Kentucky.

6. Consumer/Buyer has read this liability waiver and assumption of risk and fully understand its terms.
By making a purchase, or consuming these products, consumer understands that they are giving up their
right to sue.
